A fill-in matrix that maps each role or task to the credential it requires, the issuer, the validity period, the renewal lead time, the alert layers and the evidence you will accept.

Requirements should attach to roles and tasks, not to individuals. A person does not need a forklift certification; the task of operating a forklift requires one. This worksheet is where that definition lives. One row per role and credential gives you the matrix that head office defines and each site manager executes, and it is the mapping that turns a list of dates into a management system. Fill it in before importing a single person into a tracking tool, because the tool can only enforce what the matrix says.
The example rows show the level of detail that works: the credential named as the issuer writes it, a validity period you have confirmed rather than assumed, a renewal lead time based on how long the class or paperwork really takes, and alert layers that fit that lead time. Replace every placeholder with your own figures and add local variations as separate rows instead of footnotes.
